Adoption Application

Before submitting this application, please determine if you meet these minimum requirements:

• Please be AT LEAST 18 years of age
• Have verifiable identification
• Be able to verify that you can have a pet where you live
• Are willing to undergo a home check as part of the adoption process

Kitty Devore Rescue Network reserves the right to deny the adoption application of any pet for any reason.

Failure to provide correct information may result in adoption application denial.

If approved to adopt, I agree to provide nutritious food, fresh water, appropriate shelter, medical care as well as regular exercise and grooming for the cat I am interested in adopting once he or she has been officially adopted into my care.

If approved to adopt, I will take my adopted cat to a veterinarian for exams, illnesses and immunizations as needed and annually at my own expense.

If approved to adopt, I understand that Kitty Devore Rescue Network cannot guarantee the health, temperament or training of the KD cat I am interested in adopting.

If approved to adopt, I understand that if a health issue arises with my adopted cat within the first 30 days of adoption, I will immediately contact the Kitty Devore rescue at admin@kittydevorerescue.org and relay the problem observed if I expect the rescue to cover the expense. I will allow 24 hours for a Kitty Devore Rescue administrator to respond. Should Kitty Devore Rescue determine the situation described to be a minor health issue that may have existed undetected before adoption, I understand Kitty Devore Rescue will advise I take the cat to a rescue-friendly veterinary hospital or clinic of their choosing if I expect the rescue to reimburse the expense. I understand that if the health concern involves an injury or other health issue incurred post-adoption, Kitty Devore Rescue will not be liable for the cost. If Kitty Devore Rescue determines the situation to be a more serious concern that existed undetected before adoption, I understand the rescue will require I grant them temporary custody of my adopted cat so they can seek veterinary care and treatment at a veterinary facility of their choosing and at their expense. In serious instances, Kitty Devore Rescue will keep me apprised of my cat’s care daily and convey a projected return date within a week of the temporary re-surrender date.

If approved to adopt, I understand Kitty Devore Rescue will not reimburse me for veterinary expenses I have incurred within the first 30 days from the date of adoption if I have not communicated the health concern at the time of its observance and waited 24 hours for a response and directions from a representative of the rescue. If I decide to seek veterinary care at a veterinary facility of my own choosing without communicating the issue to the rescue and waiting for a response from a rescue representative within the 24-hour time frame, the rescue will not be liable for any expenses I have incurred.

If approved to adopt, I understand Kitty Devore Rescue will not reimburse for expenses incurred within the first 30 days from the date of adoption if they are a result of injury in the adoptive home or possible negligence on the part of the adopter.

If approved to adopt, I understand any photographs or videos taken at the time of adoption and any videos or photographs provided to any member of the Kitty Devore Rescue team post-adoption may be used on our social media platforms, ie. Facebook, Instagram and/or Twitter, or on our website. I also understand I may opt out by selecting the opt out option on this application.

If approved to adopt, I understand that after 30 days and the transfer of the kitty’s microchip from Kitty Devore Rescue to me, Kitty Devore is no longer responsible for medical expenses or reimbursement for expenses unless by mutual agreement between a Kitty Devore Rescue administrator and me prior to the 30 day mark.

If approved to adopt, I agree to license my adopted KD cat in compliance with the laws and ordinances enforced in the municipality in which I reside.

If approved to adopt, I agree not to allow my adopted KD cat to be used for medical or other experimental purposes.

If approved to adopt, I will not declaw this cat.

If approved to adopt, I understand that I am adopting a cat that has been sterilized, tested for FeLV/FIV, dewormed, has received from one to three FVRCP vaccines depending on age and has been microchipped. I understand this cat may have received a rabies ONLY if certain veterinary, transport or adoption venues required it. I understand that Kitty Devore Rescue Network does not recommend or administer the FeLV, FIV or FIP vaccines and should I want to have my adopted KD cat vaccinated with these vaccines, it will be at my own discretion and expense.

If approved to adopt, I understand the cat I have adopted has been microchipped with a microchip that can be read by a universal microchip reader. I understand the microchip number will be provided to me in the mailed adoption package sent to me within two weeks of the date of adoption. I understand the microchip will be transferred to me from Kitty Devore Rescue when 30 days have elapsed from the date of adoption. I understand it is my responsibility to register the chip in my own name, keep the information updated should I relocate and that Kitty Devore be retained on the microchip as a secondary contact.

If approved to adopt, I agree to notify Kitty Devore Rescue if I decide at any time that I can no longer keep the KD cat I have adopted or if he or she should become lost or if he or she dies.

If approved to adopt, I agree to return my adopted KD cat to Kitty Devore Rescue if I am no longer able to care for him or her. I also agree to pre-arrange with a family member or friend that if something should happen to me that disallows me to care for my adopted KD cat that they will contact a representative of Kitty Devore Rescue to make arrangements to take custody of the cat. Should the need to relinquish ownership of my adopted KD cat arise, the rights of ownership of the cat will revert back to Kitty Devore Rescue.

I give permission to Kitty Devore Rescue to contact my veterinarian to obtain information about past and present animals in my care. I also authorize Kitty Devore Rescue Network to verify the information I have provided in this application.

If, after I have adopted this cat, he or she is lost or winds up in a shelter and Kitty Devore Rescue Network representatives are contacted to retrieve him or her, I will be notified and a Kitty Devore Rescue representative will discuss the situation with me. However, contingent upon the circumstances, this cat may revert back to the custody and ownership of Kitty Devore Rescue if negligence is determined. In such instances, this contract will be null and void and the adoption fee will not be refunded.

I have read this contract and completely understand and accept the information, rights and obligations involved and agree to comply with each of them. I certify the above answers to be true and correct.

If my application is approved and accepted and contingent on an approved home check, I hereby do accept possession of, title to and responsibility for the cat identified above and release and discharge Kitty Devore Rescue from liability for any injury or damages to any person or property caused in the future by the cat and from any cause of action, claim, suits or demands that may arise as a result if any injury or damage occurs.

I understand the minimum adoption donation for the KD cat or cats I am interested in adopting is $200.00 per cat or $375.00 for two cats. The two-cat discount is only applicable if two cats are adopted at the same time or within 30 days of one another. Adoption fees are non-refundable after 30 days from the date of adoption. Each subsequent kitty adopted within 30 days of the first will receive a $25 discount off the $200.00 minimum adoption donation fee.

I understand if I am approved to adopt and after I have undergone a Meet and Greet with the kitty or kitties I am interested in adopting and passed a home check conducted by a Kitty Devore Rescue representative, I can pay the adoption fee in advance of the adoption delivery date. Doing so will secure my commitment to adopt and the kitty will be marked as "Adoption Pending" on the Kitty Devore Rescue website and Petfinder accounts so that no other prospective adopter may adopt him or her.

I understand if I do not wish to pay the adoption fee in advance, the kitty I'm interested in will remain available and open for adoption to other approved applicants. Kitty Devore Rescue cannot hold a cat indefinitely for a prospective adopter beyond one week without an adoption fee deposit of half or more of the adoption fee. If less than the full adoption fee is paid as a deposit, the balance will be due at the time the kitty is delivered to my home by a Kitty Devore Rescue representative.

We accept the following payment methods: PayPal, Zelle, or personal check. We do not accept payment in cash or by money order.

If you prefer to pay by PayPal, visit your PayPal account and use the recipient email address: admin@kittydevorerescue.org. Please include an additional $5.00 to cover the PayPal fee deduction for a total of $205.00 for one cat and $380.00 for two.

We also accept payment by Zelle by using the email address: kittydevorerescue@gmail.com or admin@kittydevorerescue.org.

Payment can also be made by check given to a Kitty Devore Rescue Network representative at the time of adoption and made payable to “Kitty Devore Rescue”.

Please do not send adoption fee payment until your application has been approved by one of our adoption officers, you've had your Meet and Greet, and have undergone a home check. Once all three aspects of our adoption process are completed and you have decided you're committed to adopting, we will happily accept your adoption fee payment - even if the decided upon delivery date is two weeks or more in the future from the date you've indicated you're committed to adopting. Adoption fees sent in advance of these three aspects of our adoption process will be returned.
